- Raipur/CG Police Lost Property (LPR) certificate copy
- Form 26 (with chassis pencil print impressions)
- Notarized lost RC sworn affidavit verified by Chhattisgarh legal desk
- Original RC card copy (if available) or details showing Chassis/Engine number
- Valid Comprehensive/Third-party insurance policy certificate
